90-Day Benefit Enrollment for
Newly Hired Crowley Employees
Voluntary Employee-Paid Benefits

Crowley is excited to include voluntary employee‐paid benefits offered through Allstate Benefits and enrolled by U.S. Enrollment Services. Crowley will set up automatic post‐tax pay‐roll deductions for these benefits which will be effective the first of the month after you enroll.

These benefits are offered on a Guarantee Issue basis. This means you are eligible to elect any of the plans, even if you have a pre-existing condition*. The Guarantee Issue is only offered for this 90-Day Enrollment period. After the enrollment period ends, you will be required to answer health questions and be qualified by the carrier, Allstate Benefits, in order to participate.

*It is important that you schedule time to speak one-on-one with a Benefit Specialist from U.S. Enrollment Services to receive detailed information about these benefits. Your individual meeting will take approximately 15-20 minutes.

Accident Insurance

Benefits offered by this policy correspond with: medical expenses including emergency room, hospital confinement, ambulance, dislocation or fracture, loss of life or limb, and disability income protection (for the primary insured only).

Critical Illness

Critical Illness insurance pays a lump-sum benefit for specified critical illnesses, such as heart attack, cancer, end stage kidney failure, coronary artery bypass surgery and stroke. The policy can help employees pay indirect costs of treatment, such as home health care, and modifications to a home or car to accommodate a wheelchair and other special needs.

Universal Life

Universal Life Insurance is a permanent insurance plan. This means, regardless of health, it will remain in force and the rate will never increase for as long as the insured lives and the premiums are paid.

These benefits will be offered on a Guarantee Issue basis. This means you are eligible to elect any of the plans, even if you have a pre-existing condition*. The Guarantee Issue is only offered for this 90-Day Benefit period. After this enrollment, you will be required to answer health questions and be qualified by the carrier, Allstate Benefits, in order to participate.

How Do I Schedule an Appointment?

U.S. Enrollment Services will be facilitating the enrollment process with each employee. Because these special underwriting concessions have been made to offer Guarantee Issue during this enrollment period, U.S. Enrollment Services will need to collect an election or waiver for all employees.
